You should complete this form if one parent or guardian cannot go with the child to apply in person for the child’s passport. The parent or guardian should submit this special form in addition to their child’s application (DS-11).

Adult Passport Renewal (DS-82)
You should complete this form if you meet our requirements to renew your passport. Keep in mind that children under age 16 cannot renew their passports and must apply using Form DS-11.
Report and Replace Lost/Stolen Passport (DS-64 & DS-11)
You should complete this form if you lost your passport or it has been stolen. To replace a lost or stolen passport, you must apply in person and submit Form DS-11 in addition to Form DS-64.

The U.S. Embassy in Dhaka encourages all passport applicants to read the advisory on their passport applications about the requirement to provide Social Security Numbers on all applications for U.S. passports. This requirement, under Section 6039E of the Internal Revenue Code, makes the failure to provide a valid Social Security Number when applying for a U.S. passport or for the renewal of a U.S. passport an offense punishable by a $500 fine. The Internal Revenue Service (IRS) points out that a U.S. citizen, if their income is over a certain threshold, has an obligation to file a U.S. tax return, and in order to file that return a Social Security Number is required. American citizens without a valid Social Security Number should see the Federal Benefits section of the ACS website for information on how to apply for a Social Security Number.
