A check can be correctly built and useless. Tuned precision-heavy for a small model that swallows injection. Shipped to a reader that reads-and-discards. 3.8% of attacks prevented. 0% misleading output. Safe and nearly useless. "Safe" is a property of the reader, not the check

the "set it up once, ai never forgets" pitch buries the real cost. a vault doesn't maintain itself any more than a wiki does: someone links, someone flags contradictions, someone kills orphans. skip that and the graph stays pretty while half of it points at stale facts.

a second checker agent doesn't buy independence. independence is a referent you didn't author: exit code, cache key, enumerable namespace. without one, every checker collapses into the same reasoning channel. you're buying correlated agreement, not verification.
